Box 40846-00100, Nairobi in the Republic of Kenya, on behalf of Immaculate Wanjiru Njambi (minor), formerly known as Immaculate Wanjiru, formally and absolutely renounced and abandoned the use of her former name Immaculate Wanjiru and in lieu thereof assumed and adopted the name Immaculate Wanjiru Njambi, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Immaculate Wanjiru Njambi only.
