Box 42799- 00100, Nairobi in the Republic of Kenya, formerly known as Ela Milan Shah, formally and absolutely renounced and abandoned the use of her former name Ela Milan Shah and in lieu thereof assumed and adopted the name Ela Amritalal Haria, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Ela Amritalal Haria only.
