Box 724– 00206, Nairobi in the Republic of Kenya, formerly known as Michelle Liz Mishi O Ponda, formally and absolutely renounced and abandoned the use of her former name Michelle Liz Mishi O Ponda and in lieu thereof assumed and adopted the name Michelle Liz Mishi Ponda, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Michelle Liz Mishi Ponda only.
