Box 2626–60200, Meru in the Republic of Kenya, formerly known as Jolline Gakii Amtana Kamoyani, formally and absolutely renounced and abandoned the use of her former name Jolline Gakii Amtana Kamoyani and in lieu thereof assumed and adopted the name Jolline Gakii Nkari, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Jolline Gakii Nkari only.
