Box 114, Thika in the Republic of Kenya, formerly known as Coleta Nyambura Kamau, formally and absolutely renounced and abandoned the use of her former name Coleta Nyambura Kamau, and in lieu thereof assumed and adopted the name Coleta Nyambura Muhia, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Coleta Nyambura Muhia only.
