Box 511–10101, Karatina in the Republic of Kenya, on behalf of John Wesley Baraka (minor), formerly known as John Wesly Miano, formally and absolutely renounced and abandoned the use of his former name John Wesly Miano and in lieu thereof assumed and adopted the name John Wesley Baraka, for all purposes and authorizes and requests all persons at all times to designate, describe and address him by his assumed name John Wesley Baraka only.
