Box 684- 40200, Kisii in the Republic of Kenya, formerly known as Anne Kwamboka Ayienda, formally and absolutely renounced and abandoned the use of her former name Anne Kwamboka Ayienda and in lieu thereof assumed and adopted the name Anne Kwamboka Ogwora, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Anne Kwamboka Ogwora only.
