Box 38667–00623, Nairobi, formerly known as Chandangaury Bhupendra Ankeri Desai, formally and absolutely renounced and abandoned the use of her former name Chandangaury Bhupendra Ankeri Desai and in lieu thereof assumed and adopted the name Chandangaury Bhupendra Desai, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Chandangaury Bhupendra Desai only.
