GAZETTE NOTICE NO. 6435
NOTICE is given that evidence of loss or destruction of the above
policy document has been submitted to the company and any person in
possession of it or claiming to have interest therein should
communicate within thirty (30) days by registered post with the
company, failing any such communication a certified duplicate copy of
the policy document which shall be the sole evidence of the contract
will be issued.
