Box 135– 20306, Ndaragwa in the Republic of Kenya, formerly known as Ahaziah Nyatha Gitahi, formally and absolutely renounced and abandoned the use of my former name Ahaziah Nyatha Gitahi, and in lieu thereof assumed and adopted the name Moses Ahaziah Nyatha Gitahi, for all purposes and authorizes and requests all persons at all times to designate, describe and address me by my assumed name Moses Ahaziah Nyatha Gitahi only.
