Box 85810–80100, Mombasa in the Republic of Kenya, formerly known as Natalie Akinyi Sellah Ogada, formally and absolutely renounced and abandoned the use of my former name as Natalie Akinyi Sellah Ogada and in lieu thereof assumed and adopted the name Natalie Akinyi Ogada, for all purposes and authorizes and requests all persons at all times to designate, describe and address me by my assumed name Natalie Akinyi Ogada only.
