Box 4123–00100, Nairobi in the Republic of Kenya, on behalf of Wayne Kiprop Arusei (minor), formerly known as Wayne Okiru Orone, formally and absolutely renounced and abandoned the use of his former name Wayne Okiru Orone, and in lieu thereof assumed and adopted the name Wayne Kiprop Arusei, for all purposes and authorizes and requests all persons at all times to designate, describe and address him by his assumed name Wayne Kiprop Arusei only.
