Box 3897– 00200, Nairobi in the Republic of Kenya, formerly known as Stacy Nduta Joseph, formally and absolutely renounced and abandoned the use of her former name Stacy Nduta Joseph and in lieu thereof assumed and adopted the name Stacy Nduta Karczewska, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Stacy Nduta Karczewska only.
