Box 90184–80100, Mombasa in the Republic of Kenya, formerly known as Gladys Asha Nzisha, formally and absolutely renounced and abandoned the use of my former name Gladys Asha Nzisha and in lieu thereof assumed and adopted the name Gladys Nzisa Ibrahim, for all purposes and authorizes and requests all persons at all times to designate, describe and address me by my assumed name Gladys Nzisa Ibrahim only.
