Box 15550–00100, Nairobi in the Republic of Kenya, formerly known as Yvonne Amisi Omondi, formally and absolutely renounced and abandoned the use of her former name Yvonne Amisi Omondi, and in lieu thereof assumed and adopted the name Yvonne Amisi, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Yvonne Amisi only.
