Box 16008–00610, Nairobi in the Republic of Kenya, on behalf of Aiden Israel Omondi (minor), formerly known as Aiden Visha Owuor Omondi, formally and absolutely renounced and abandoned the use of his former name Aiden Visha Owuor Omondi, and in lieu thereof assumed and adopted the name Aiden Israel Omondi, for all purposes and authorizes and requests all persons at all times to designate, describe and address him by his assumed name Aiden Israel Omondi only.
