Box 1472–30100, Eldoret in the Republic of Kenya, formerly known as Mark Kiprotich, formally and absolutely renounced and abandoned the use of my former name Mark Kiprotich and in lieu thereof assumed and adopted the name Mark Meli Too for all purposes and authorizes and requests all persons at all times to designate, describe and address me by my assumed name Mark Meli Too only.
