Box 24444– 00502, Nairobi in the Republic of Kenya, formerly known as Keshavjee Tarek Farid alias Tarek Farid Lutaf, formally and absolutely renounced and abandoned the use of her former name Keshavjee Tarek Farid alias Tarek Farid Lutaf and in lieu thereof assumed and adopted the name Tarek Farid Keshavjee, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Tarek Farid Keshavjee only.
