Box 970–00621, Westlands in the Republic of Kenya, formerly known as Lokwar Naomi Kwamboka Kabinga Lukiri, formally and absolutely renounced and abandoned the use of her former name Lokwar Naomi Kwamboka Kabinga Lukiri and in lieu thereof assumed and adopted the name Lokwar Naomi Kwamboka, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Lokwar Naomi Kwamboka only.
