Box 76656–00508, Nairobi in the Republic of Kenya, on behalf of Violet Maria Dobrzykowski (minor), formerly known as Violet Osato Osawaru, formally and absolutely renounced and abandoned the use of her former name Violet Osato Osawaru and in lieu thereof assumed and adopted the name Violet Maria Dobrzykowski, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Violet Maria Dobrzykowski only.
