Box 44476- 00100, Nairobi in the Republic of Kenya, formerly known as Betty Wanjiku Kamau, formally and absolutely renounced and abandoned the use of her former name Betty Wanjiku Kamau in lieu thereof assumed and adopted the name Betty Wanjiku Kamau Kanaiya, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Betty Wanjiku Kamau Kanaiya only.
