Box 60286-00200, Nairobi in the Republic of Kenya, formerly known as Andrew Okiya Omtatah alias Andrew Okoiti alias Andrew Omtatah Okoiti, formally and absolutely renounced and abandoned the use of his former name Andrew Okiya Omtatah alias Andrew Okoiti alias Andrew Omtatah Okoiti and in lieu thereof assumed and adopted the name Andrew Okiya Omtatah Okoiti, for all purposes and authorizes and requests all persons at all times to designate, describe and address him by his assumed name Andrew Okiya Omtatah Okoiti only.
