Box 1209–00232, Ruiru in the Republic of Kenya, on behalf of Leo Mwadime Epey (minor), formerly known as Leo Mwawasi Epey, and absolutely renounced and abandoned the use of his former name Leo Mwawasi Epey, formally and in lieu thereof assumed and adopted the name Leo Mwadime Epey, for all purposes and authorizes and requests all persons at all times to designate, describe and address him by his assumed name Leo Mwadime Epey only.
