Box 717–90200, Kitui in the Republic of Kenya, formerly known as Winnie Katethya Musaili Kasyula, formally and absolutely renounced and abandoned the use of my former name Winnie Katethya Musaili Kasyula and in lieu thereof assumed and adopted the name Winnie Katethya Makau for all purposes and authorizes and requests all persons at all times to designate, describe and address me by my assumed name Winnie Katethya Makau only.
