Box 57720–00200, Nairobi in the Republic of Kenya, on behalf of Leila Amani Cheruiyot (minor), formerly known as Leila Amani, formally and absolutely renounced and abandoned the use of her former name Leila Amani and in lieu thereof assumed and adopted the name Leila Amani Cheruiyot for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Leila Amani Cheruiyot only.
