Box 16, Edimburg Dr, Glassgow, G40 4RW, in the United Kingdom, formerly known as Jane Nduta Gathogo, formally and absolutely renounced and abandoned the use of her former name Jane Nduta Gathogo, and in lieu thereof assumed and adopted the name Jennifer Muthoni Collins for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Jennifer Muthoni Collins only.
