Box 99382– 80107, Mombasa in the Republic of Kenya, formerly known as Valentine Mwaisaka Mwakamba, formally and absolutely renounced and abandoned the use of my former name Valentine Mwaisaka Mwakamba and in lieu thereof assumed and adopted the name Brazil Mwaisaka Mwakamba, for all purposes and authorizes and requests all persons at all times to designate, describe and address me by my assumed name Brazil Mwaisaka Mwakamba only.
