Box 93–30129, Chepkorio, Elgeyo Marakwet County in the Republic of Kenya, formerly known as Gilbert Kipkemoi Sugut, formally and absolutely renounced and abandoned the use of his former name Gilbert Kipkemoi Sugut, and in lieu thereof assumed and adopted the name Tony Gilbert Sugut, for all purposes and authorizes and requests all persons at all times to designate, describe and address him by his assumed name Tony Gilbert Sugut only.
