Box 30– 60100, Embu in the Republic of Kenya, formerly known as Nyaga Njogu alias Kithaka Njogu, formally and absolutely renounced and abandoned the use of my former name Nyaga Njogu alias Kithaka Njogu, and in lieu thereof assumed and adopted the name Cypriano Nyaga Njogu, for all purposes and authorizes and requests all persons at all times to designate, describe and address me by my assumed name Cypriano Nyaga Njogu only.
