Box 54674–00200, Nairobi in the Republic of Kenya, on behalf of Vallary Damilola Nduta Akinwumi (minor), formerly known as Vallary Damilola Nduta Akinwumi alias Vallary Damilola Akinwumi, formally and absolutely renounced and abandoned the use of her former name Vallary Damilola Nduta Akinwumi alias Vallary Damilola Akinwumi and in lieu thereof assumed and adopted the name Vallary Damilola Nduta Akinwumi, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Vallary Damilola Nduta Akinwumi only.
