Box 40002– 80100, Mombasa in the Republic of Kenya, on behalf of Khalid Mohamed Said Bakhresa (minor), formerly known as Khalid Mohamed Saidi Bakhresa, formally and absolutely renounced and abandoned the use of his former name Khalid Mohamed Saidi Bakhresa, and in lieu thereof assumed and adopted the name Khalid Mohamed Said Bakhresa, for all purposes and authorizes and requests all persons at all times to designate, describe and address him by his assumed name Khalid Mohamed Said Bakhresa only.
