Box 54840–00200, Nairobi in the Republic of Kenya, formerly known as Cynthia Nakwekwe Nekesa Wangila, formally and absolutely renounced and abandoned the use of her former name Cynthia Nakwekwe Nekesa Wangila, and in lieu thereof assumed and adopted the name Cynthia Nakwekwe Wangila, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Cynthia Nakwekwe Wangila only.
