Box 327– 30400, Kabarnet in the Republic of Kenya, formerly known as Mercy Jerop, formally and absolutely renounced and abandoned the use of her former name Mercy Jerop and in lieu thereof assumed and adopted the name Ursila Jerop Kimutai, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Ursila Jerop Kimutai only.
