Box 00625– 29049, Nairobi in the Republic of Kenya, formerly known as Carolime Kalume, formerly and absolutely renounced and abandoned the use of her former name Carolime Kalume, and in lieu thereof assumed and adopted the name Carolime Kalume Cheupe for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Carolime Kalume Cheupe only.
