Box 13411, Mombasa in the- Republic of Kenya, formerly known as Loci Mkawughanga Mwamba Mutio, formally and absolutely renounced and abandoned the use of her former name Loice Mkawughanga Mwamba Mutio, and in lieu thereof assumed and adopted the name Loice Mkawughanga Mwamba, for all purposes and authorizes and requests all at all times to designate, describe and address her by her assumed name Loice Mkawughanga Mwamba only.
