Box 22, Kiritiri in the Republic of Kenya, formerly known as Njiru Nyaga, formally and absolutely renounced and abandoned the use of my former name Njiru Nyaga, and in lieu thereof assumed and adopted the name Josphat Kariuki Nyaga, for all purposes and authorize and request all persons at all times to designate, describe and address me by my assumed name Josphat Kariuki Nyaga only.
