Box 57676-00200, Nairobi in the Republic of Kenya, formerly known as Rose Pauline Mugure Kabugua, formally and absolutely renounced and abandoned tfie use of my former name Rose Pauline Mugure Kabugua, and in lieu thereof assumed and adopted the name Mugure Kabugua Mugo, for all purposes and authorize and request all persons at all times to designate, describe and address me by my assumed name Mugure Kabugua Mugo only.
