Box 61011– 00200, Nairobi in the Republic of Kenya, the mother, guadian and next friend of Zoe Njeri Mputhia, a minor formerly known as Zoe Njeri Kahiri, formerly known as Zoe Njeri Kahiri, formally and absolutely renounced and abandoned the use of her former name Zoe Njeri Kahiri, and in lieu thereof assumed and adopted the name Zoe Njeri Mputhia, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Zoe Njeri Mputhia only.
