Box 3246, Abiaka Drive, Kissimmee, FL, 34743, U.S.A., a citizen of the Republic of Kenya, formerly known as Mutio Mutinda, formally and absolutely renounced and abandoned the use of her former name Mutio Mutinda, and in lieu thereof assumed and adopted the name Serah Mutio Alphonce, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Serah Mutio Alphonce only.
