Box 40 West, 116th Street, Newyork, NY 10026, in the United States of America, formerly known as Priscilla Atwani Akwara alias Priscilla Sabina Atwani Akwara alias Priscilla Sabina Atwani Idele, formally and absolutely renounced and abandoned the use of her former names Priscilla Atwani Akwara alias Priscilla Sabina Atwani Akwara alias Priscilla Sabina Atwani Idele, and in lieu thereof assumed and adopted the name Priscilla Atwani Idele, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Priscilla Atwani Idele only.
