Box 519, Mombasa in the Republic of Kenya, formerly known as John Opiyo Ouma, formally and absolutely renounced and abandoned the use of my former name John Opiyo Ouma, and in lieu thereof assumed and adopted the name John Bwire Ouma, for all purposes and authorize and request all persons at all times to designate, describe and address me by my assumed name John Bwire Ouma only.
