Box 82991, Mombasa in the Republic of Kenya, formerly known as Khushi Dhiraj Premji Naran, formally and absolutely renounced and abandoned the use of my minor son‘s former name Khushi Dhiraj Premji Naran, and in lieu thereof assumed and adopted the name Khushi Dhiraj Halai, for all purposes and authorizes and requests all persons at all times to designate, describe and address my said minor son by his assumed name Khushi Dhiraj Halai only.
