Box 82991, Mombasa in the Republic of Kenya, formerly known as Preet Dhiraj Premji Naran, formally and absolutely renounced and abandoned the use of my minor daughter‘s former name Preet Dhiraj Premji Naran, and in lieu thereof assumed and adopted the name Preet Dhiraj Halai, for all purposes and authorizes and requests all persons at all times to designate, describe and address my said minor daughter by her assumed name Preet Dhiraj Halai only.
