Box 10328–00400, Nairobi in the Republic of Kenya, formerly known as Lucy Wambui Matathia, formally and absolutely renounced and abandoned the use of my former name Lucy Wambui Matathia, and in lieu thereof assumed and adopted the name Lucy Wambui Gachomba, for all purposes and authorize and request all persons at all times to designate, describe and address me by my assumed name Lucy Wambui Gachomba only.
