NOTICE is given that by a deed poll dated 8th July, 2011, duly executed and registered in the Registry of Documents at Nairobi as Presentation No. 2986, in Volume DI, Folio 172/4005, File No. MMXI, by our client, Ravilal Keshra Patel, of P.O. Box 880–60200, Meru in the Republic of Kenya, the father and legal guardian of Dhvani Ravilal Patel, formerly known as Neha Ravilal Patel, formally and absolutely renounced and abandoned the use of her former name Neha Ravilal Patel, and in lieu thereof assumed and adopted the name Dhvani Ravilal Patel, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Dhvani Ravilal Patel only.
