Box 46, Kabati, Kitui in the Republic of Kenya, formerly known as Geoffrey Vundi Nzioka, formally and absolutely renounced and abandoned the use of my former name Geoffrey Vundi Nzioka, and in lieu thereof assumed and adopted the name Vundi Nzioka, for all purposes and authorize and request all persons at all times to designate, describe and address me by my assumed name Vundi Nzioka only.
