Box 20194–00100, Nairobi in the Republic of Kenya, formerly known as Betty Atieno Fanuel, formally and absolutely renounced and abandoned the use of my former name Betty Atieno Fanuel, and in lieu thereof assumed and adopted the name Betty Atieno Owino, for all purposes and authorize and request all persons at all times to designate, describe and address me by my assumed name Betty Atieno Owino only.
