Box 2020– 80314, Chumvini in the Republic of Kenya, formerly known as Patrick King’oo Muteti, formally and absolutely renounced and abandoned the use of my former name Patrick King’oo Muteti, and in lieu thereof assumed and adopted the name Patrick Mutua King’oo Muteti, for all purposes and authorize and request all persons at all times to designate, describe and address me by my assumed name Patrick Mutua King’oo Muteti only.
