Box 11244–00400 , Nairobi in the Republic of Kenya, formerly known as Edwin Mwangi Kagwa Karugu, formally and absolutely renounced and abandoned the use of my former name Edwin Mwangi Kagwa Karugu, and in lieu thereof assumed and adopted the name Edward Mwangi Karugu, for all purposes and authorize and request all persons at all times to designate, describe and address me by my assumed name Edward Mwangi Karugu only.
