Box 3331– 01000, Thika in the Republic of Kenya, formerly known as Stephen Kamande Mwaura, formally and absolutely renounced and abandoned the use of his former name Stephen Kamande Mwaura, and in lieu thereof assumed and adopted the name Stephen Njuguna Mwaura, for all purposes and authorize and request all persons at all times to designate, describe and address him by his assumed name Stephen Njuguna Mwaura only.
